The Information We Collect & How It Is Used

  • Identifying Information: When you register to complete the Designing Healthy Environments at Work online assessment, we collect the following personal identifying information:

    • Your primary contact’s name
    • Your primary contact’s email address
    • Your primary contact’s phone number
    • Your worksite’s postal address

In addition to using this information to register your worksite for the online DHEW, we use your email address and name to communicate any updates or reminders related to your DHEW and its completion. For example, we send an email to confirm your registration and to confirm that we have received your completed DHEW data. We use your phone number as a back up for contacting you (we generally use email as our first method) with any important information regarding your DHEW. We may also use your phone number to respond to any questions you may ask that are more easily answered via phone. We use your worksite’s postal address to confirm your worksite's identity (as many worksites have similar names) or if you request that something related to the DHEW be mailed to you. Your personal identifying information will never be shared with anyone except the DHEW Support team unless we contact you and obtain your permission. It will be used only for the purpose of administering this website (including the online DHEW forms) and effectively communicating with you and your team about issues related to your DHEW form and about news related to the Healthy Work Environments website.

  • Information About Your Worksite's Environment: Via the DHEW form, we also collect information about various aspects of your work environment that environment that are useful in assessing the healthiness of your worksite’s environment. It will take about 2-4 hours to collect the information needed to complete the DHEW and 30 to 60 minutes to enter the data online.

The responses you provide on your DHEW will be summarized, along with responses from other worksites, with no identification of your name or other identifiers, which would connect responses with individuals or worksites.

Individual worksite level responses may occasionally be reported but will not be associated with any information that would identify the worksite or the individuals who completed the form, unless you provide permission. Your worksite will also be given a feedback report summarizing your DHEW data for use by you, your worksite and your team. Your feedback report will be delivered only to your team, and it is your decision about whether to share it with others.

Please note that all worksites that complete the DHEW assessment will be publicly recognized by placing them on a map of Michigan Health Tools Assessment participants (at www.mihealthtools.org. We will publicize only the fact that your worksite has participated in the DHEW, not your worksite’s scores nor your worksite’s responses. We may also publicize your DHEW participation through other channels, such as press releases, presentations, and on other Michigan Health Tools websites.

Completion of the DHEW is voluntary. Choosing not to complete the DHEW will not affect the services you or your worksite receive from the sponsoring organizations.
