MAINTAINING CURRENT ASTHMA ACTION PLANS FOR STUDENTS WITH ASTHMA SUCCESS STORY
All students with asthma, Kindergarten through 12th grade, were identified by reported emergency card information. Individualized care plans were compiled with input from the student, nurse, parent and physician. Medication orders were obtained from the doctor for any student requiring an inhaler or medication at school. Emergency information was obtained from the parent and the information is kept on file in the main office, and in the nurse’s office. Care plans were also given to each teacher. Triggers for individual students were identified. An Asthma Action Plan was compiled for each student, listing Green(Go), Yellow (Caution), and Red (Danger) zones for each student.
