Introduction & Overview

The sponsoring partner organizations (Michigan Team Nutrition, Michigan Department of Education, United Dairy Industry of Michigan, Michigan Department of Community Health, Michigan State University Extension, and the American Cancer Society) are committed to protecting your privacy and to making sure you know about the privacy policies and practices associated with the Healthy School Action Tools (HSAT) Assessment and Action Plan and this website. This page discloses the privacy practices for this website and for the online HSAT.

The Information We Collect & How It Is Used

  • Identifying Information: When you register to complete the Healthy School Action Tools, we collect the following personal identifying information:

    • Your primary contact's name
    • Your primary contact's email address
    • Your primary contact's phone number
    • Your school's postal address

    In addition to using this information to register your school for the online HSAT, we use your email address and name to communicate any updates or reminders related to your HSAT and its completion. For example, we send an email to confirm your registration and to confirm that we have received your completed HSAT data. We use your phone number as a back up for contacting you (we generally use email as our first method) with any important information regarding your HSAT. We may also use your phone number to respond to any questions you may ask that are more easily answered via phone.We use your school's postal address to confirm your school's identity as many schools have similar or identical names. If you request that something related to the HSAT be mailed to you via the school, we will also use the school's postal address for that purpose.

    Your personal identifying (name and email address) will be posted on this website in conjunction with your school name.We provide this information for schools that have registered or completed the HSAT so other school and community leaders (e.g. staff in your building, district superintendent, school board members, administrators or staff from other schools, or staff at community organizations like your local health department) can contact you if they have questions.

  • Information About Your School's Environment: Via the HSAT online forms, we also collect information about various aspects of your school environment that pertain to asthma management, physical activity, healthy eating and tobacco use prevention. The purpose of completing the HSAT Assessment and the planning component that accompanies it is to facilitate a review of the health environment at your school so improvements can be made. Depending on the number of topic areas you choose to address, it will take several hours to meet with your school team and research the answers for the HSAT Assessment. Entering your answers on the online assessment will take less time - usually not more than 2 hours - and can be done over as many sessions as you like.The HSAT Action Plan is an ongoing process that also involves collaborative work with your school team and therefore, determining which actions should be taken and implementing those actions can take varying amounts of time. Updating the online action plan with information or completion dates takes a few minutes for each planned action.

    The responses you provide on your HSAT will be available to the HSAT administrative staff and the partner organizations to enable them to provide you with the necessary technical support.Otherwise, your responses will be summarized, along with responses from other schools, with no identification of your name or other identifiers which would connect responses with individuals or schools.

    Individual school level responses may occasionally be reported or shared but will not be associated with any information that would identify the school or the individuals who completed the form, unless you provide permission or if we are contacted with a Freedom of Information Action request. Your school may also be given a feedback report summarizing your HSAT data for use by you, your school and your team. Your feedback report will be delivered only to your team; it is your decision about whether to share it with others.

    Please note that the Michigan Healthy Communities Collaborative will be publicly recognizing all schools that complete the HSAT Assessment by placing them on a map of Michigan Health Tools Assessment Participants (at www.mihealthtools.org) starting in 2008. Only the fact that schools have participated in the HSAT Assessment will be published, not schools' scores or schools' responses. We may also publicize HSAT Assessment participation through other channels, such as press releases, presentations, and on other Michigan Health Tools websites.

    Completion of the HSAT is voluntary. Choosing not to complete the HSAT will not affect the services you or your school receive from the sponsoring organizations.

How Your Information Is Protected

This site has security measures in place to protect the loss, misuse, and alteration of the information under our control. Your name, email address, phone number and school data are maintained on secured files accessible only by the HSAT team (the partner organizations sponsoring this website). However, as with any Internet endeavor, absolute security of information transmitted through channels (communication networks, hardware, and software) not directly operated or controlled by us or our partners cannot be guaranteed.

How to Update Your Information

We rely on you to provide updates to information that you have provided to us. You can update your email address, your primary contact name and phone number, your school address, team nutrition status and type of school (for example, public vs. private) by logging in with your username and password (the one you selected at the time of registration). Then click on the Update Your Info menu item and follow the instructions provided. You can also change your password using Update Your Info. Changes to the responses you provide on the HSAT Assessment can be changed any time prior to submitting it as complete by simply logging in and changing your answers. Similarly, you can change any Action Plan information any time prior to marking an action as complete. For other information that you would like to update or correct on an already submitted HSAT Assessment, please contact the Healthy Schools Technical Support Team, via email at healthy.schools@earthlink.net.

Opt Out Policy

You may stop completing the HSAT Assessment or Action Plan at any time; participation is always voluntary. All that you need to do is stop logging in and recording information. If you no longer wish to receive emails about the HSAT and the Healthy Schools/Healthy Students websites, please email healthy.schools@earthlink.net to be removed from the contact list.

Use of Cookies

In the unlikely event any substantial changes are made to this privacy policy, you will be notified via email (provided you are still on our email list). Examples of substantial changes include changes in the way your identifying information or school data will be used or shared.

Minor changes may also be made (for example, to clarify points based on user feedback or to change contact information if there are changes in administrative staff). You will not receive email notification of such minor changes; the most recent version of the policy will always appear on this website.
